Anaphylaxis incident reporting template

Template for reporting an allergic reaction cec

All allergic reactions (where there is a risk of anaphylaxis) should be notified to the Regulatory Authority by the approved provider. This can be done through the National Quality Agenda IT System (NQAITS) online reporting system and should be undertaken within 24 hours of the incident.

Documentation about the incident should include adequate details about the circumstances and the management of the reaction. Allergic reactions to packaged foods or food provided by a food service provider after the allergy has been declared, should be reported to the local Health Department.

This template is designed to help Children’s Education and Care services collect important information when a child (or staff member) has had an allergic reaction.
